Clarification and Consent Text for the Processing of Personal Data
We request your explicit consent regarding the matters listed below, except for cases where your personal data, as detailed in the Clarification Text on the Processing of Personal Data by Dentaglobal Sağlık Hizmetleri Ticaret Anonim Şirketi, are processed and transferred to the extent necessary and required in the following circumstances: the execution of a contract, legal obligations, public health protection, preventive medicine, medical diagnosis, treatment and care services, as well as for the planning and management of healthcare services and their financing.
Your personal data, including personal health data necessary for the execution of all medical diagnosis, examination, treatment, and care services, and obtained for this purpose, are listed below:
- Identity Data: Name, surname, Turkish ID number, passport number or temporary Turkish ID number (if you are not a Turkish citizen), place and date of birth, marital status, gender, and other identity details along with any personal data obtained from your presented Turkish ID card or similar identity documents.
- Contact Data: Address, phone number, email address, and other communication information.
- Financial Data: Bank account number, IBAN number, etc.
- Health Data: Medical information such as lab and imaging results, test results, examination data, prescription details, and other data related to medical diagnosis, treatment, and care obtained during the provision of healthcare services. Additionally, responses and feedback you have shared to evaluate our services.
- Digital Data: Audio and video recordings captured through the camera system during your visit to our hospitals, and voice recordings of calls with our call center.
- Other: Information related to your private health insurance and Social Security Institution data for the financing and planning of your healthcare services, browsing data obtained while using our website and mobile application, such as IP address, browser information, and medical documents, surveys, form information, and location data that you have voluntarily provided.
We inform you that your personal and sensitive personal data mentioned above can be processed for the following purposes:
- Protecting public health, preventive medicine, medical diagnosis, treatment, and care services;
- Sharing the required information with the Ministry of Health and other public institutions in accordance with the relevant legislation;
- Compliance with legal and regulatory requirements;
- Financing your healthcare services, and covering your diagnostic, treatment, and care costs, sharing the necessary information with private insurance companies for eligibility checks within the scope of healthcare services by the Patient Services, Financial Affairs, and Marketing departments;
- Informing you about your appointment through our Call Center and Digital Channels;
- Verifying your identity by the Patient Services, Healthcare Personnel, and Call Center departments;
- Planning and managing internal operations by the Hospital Management;
- Analyzing healthcare services for improvement by the Quality, Patient Experience, and Information Systems departments;
- Monitoring and preventing misuse or unauthorized transactions by the Audit and Information Systems departments;
- Performing risk management and quality improvement activities by the Quality, Patient Experience, and Information Systems departments;
- Billing your services by the Patient Services, Financial Affairs, and Marketing departments;
- Confirming your relationship with institutions contracted with the hospital by the Patient Services, Financial Affairs, and Marketing departments;
- Responding to any questions or complaints about the healthcare services provided to you by the Hospital Management, Patient Experience, Patient Rights, and Call Center departments;
- Taking all necessary technical and administrative measures for the data security of the hospital systems and applications by the Hospital Management and Information Systems departments;
- Providing participation in and information about campaigns, designing and delivering personalized content and benefits through Web and mobile channels by the Marketing, Media, Communication, and Call Center departments;
- Measuring, increasing, and researching patient satisfaction by the Hospital Management, Patient Rights, and Patient Experience departments.
We inform you in detail that the above personal and sensitive personal data may be stored in physical and electronic archives under Dentaglobal’s and external service providers’ management, with great care and in compliance with legal regulations.
As part of the Transfer of Personal Data, your personal data will be shared with:
- The Ministry of Health and affiliated subunits, family medicine centers;
- The Social Security Institution;
- The General Directorate of Security and other law enforcement agencies;
- The General Directorate of Population;
- The Turkish Pharmacists' Association;
- Judicial authorities;
- Your authorized legal representatives;
- Private insurance companies;
- Third parties, including your attorneys, tax consultants, and auditors with whom you receive consultancy services;
- Other hospitals to which you are referred or have applied independently;
- Regulatory and supervisory agencies and official authorities;
- Suppliers, service providers, archive service providers, and business partners we collaborate with or whose services we benefit from.
Method and Legal Basis for Collecting Personal Data
We inform you that your personal data are collected and processed through verbal, written, visual, or electronic means, as necessary to carry out any business related to Dentaglobal’s activities within the scope of legal regulations and to fully and appropriately perform Dentaglobal’s contractual and legal obligations.
I acknowledge that I have read and understand the information provided above, and I am aware that the legal basis for collecting my personal data by Dentaglobal is as follows:
- Law No. 6698 on the Protection of Personal Data;
- Law No. 3359 on Basic Health Services;
- The Decree-Law No. 663 on the Organization and Duties of the Ministry of Health and Affiliated Institutions;
- The Regulation on the Processing and Protection of Confidentiality of Personal Health Data;
- Health Ministry regulations and other relevant legislation.
Additionally, I understand that according to Article 6, paragraph 3 of the Law, personal data related to health and sexual life may be processed without my explicit consent only by persons or authorized institutions bound by confidentiality obligations for the purposes of protecting public health, preventive medicine, medical diagnosis, treatment and care services, and the planning and management of healthcare services and their financing.
Rights Regarding the Protection of Personal Data
I have been informed of my rights regarding the processing of my personal data, including:
- The right to know whether my personal data is being processed;
- The right to request information if my personal data has been processed;
- Access to my personal health data and requesting a copy;
- The right to learn the purpose of processing my personal data and whether it is used in line with that purpose;
- The right to know the third parties to whom my personal data is transferred domestically or abroad;
- The right to request the correction of incomplete or inaccurate personal data;
- The right to request the deletion or destruction of my personal data;
- The right to request notification of the corrections or deletions of my personal data to third parties who received my data;
- The right to object to any unfavorable outcome derived from the exclusive analysis of my personal data through automated systems;
- The right to demand compensation if I suffer damage due to unlawful processing of my personal data.
